Role description

Job summary

As an autonomous practitioner the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes. This includes physical examination and detailed history taking, diagnosis of deterioration inpre-existing conditions and treatment planning. Your experience and skills should enable you to recognise that a person is deteriorating and advanced care planning might be appropriate and/or that the person is coming into the last days of life and that dying phased care is appropriate.

Main duties of the job

The post holder will rotationally work within the Telephone response service for Palliative and End of life Care providing a 24/7 working closely with admission avoidance services to ensure patients safety is maintained.

The post holder will be the keyworker for patients within the virtual ward/virtual care. The post holder will work in this adaptable and evolving role, focusing on safe, effective admission avoidance, and supporting timely discharge from acute hospitals, with awhole systems approach. The post holder will co-ordinate care and strive to reduce the risk of acute re-admissions by identifying risks, implementing management plans, and collaborating with the whole community resource to facilitate ongoing care management
