Job summary

We have an exciting opportunity for an experienced and motivated Advanced Clinical Practitioner to join our developing East Cornwall Community Hospital and Urgent Care Services in a unique peripatetic role. Working across Liskeard, Launceston, Stratton and other East Cornwall community settings, you will play a pivotal role in delivering high-quality, person-centred care to patients with acute and complex healthcare needs, whilst supporting the transformation of integrated community services.

This autonomous and highly rewarding role will see you working across Same Day Emergency Care (SDEC), Community Assessment and Treatment Units (CATUs), Virtual Wards, Urgent Treatment Centres and Community Hospital inpatient services. You will utilise advanced assessment, diagnostic and prescribing skills to provide expert clinical leadership, support timely decision-making, and help patients receive the right care in the right place at the right time.

Main duties of the job

As an Advanced Clinical Practitioner (ACP), you will provide autonomous clinical care across Community Hospitals, Same Day Emergency Care (SDEC), Community Assessment and Treatment Units (CATUs), Virtual Wards and Urgent Treatment Services in East Cornwall. You will assess, diagnose, treat and manage patients with acute, chronic and complex conditions, independently managing admissions, investigations, prescribing and care planning within your scope of practice.

Working across inpatient and community settings, you will provide clinical leadership, support multidisciplinary teams, facilitate safe discharge planning and help reduce avoidable admissions. Using advanced decision-making skills, you will manage clinical risk, recognise deterioration and deliver timely interventions, including advanced procedures and IV therapies. You will advocate for patients, support shared decision-making, provide specialist advice, supervise colleagues and contribute to audit, research, service evaluation and quality improvement.

You will support NHS priorities and service development through mentorship, education and high standards of professional practice and record keeping. This flexible role requires travel across Liskeard, Launceston, Stratton, Bodmin and other East Cornwall healthcare settings, ensuring advanced clinical expertise is available wherever needed to support patient flow and high-quality, person-centred care.

Education / Qualifications and Relevant Experience

Essential

  • Current first level professional registration - RN Adult
  • Recognised teaching or mentorship qualification equivilant to level 3
  • Independent Non-Medical Prescriber Qualification & registration with NMC
  • Masters in Advanced Nurse Practitioner / or a verified portfolio of evidence or working towards

Skills and Aptitude

Essential

  • Strong analytical and judgement skills, with the ability to interpret complex situations and make appropriate decisions.
  • Advanced clinical practice skills such as interpretation of ECG; X-rays.
