An exciting opportunity to join our Home First Rehab & Reablement team. Our service covers the whole of Salford facilitating clients discharge back into their own home. Our vision for Home First is 'People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary'
We are seeking for caring and compassionate people who enjoys interacting and assisting people in their own homes and assisting them with their daily living activities. You will be an excellent team player and also possess excellent communication skills and must be able to work on your own initiative. Candidates must be warm and friendly, with a strong work ethic, a positive attitude and a genuine passion for providing high quality care and support for the people of Salford.
Due to the role working in the community a Full Driving Licence and the use of own vehicle is essential.
The Home First Reablement Service operates 7 days a week from 8am to 10pm and various shift working is available. The successful applicants will work on a rota that is provided in advance to cover shifts that includes weekends and Bank Holidays.
Main duties of the job
Main duties and responsibilities
- Assess, coordinate and review reablement support to promote recovery, independence and reduced reliance on long-term care.
- Complete person-centred assessments identifying needs, goals, risks and support requirements.
- Develop, implement and review individualised support plans aligned to service user outcomes.
- Liaise with service users, families, carers, providers, Adult Social Care, health professionals and community partners to support effective care.
- Coordinate and monitor visits via Malinko, ensuring timely delivery aligned to assessed need.
- Review progress with service users and providers, adapting plans as needed.
- Coach provider staff on reablement principles and strengths-based practice.
- Support safe discharge planning, identifying ongoing care needs with Adult Social Care.
- Coordinate input from pharmacy, community, voluntary organisations and other agencies.
- Arrange equipment, assistive technology and minor adaptations to support independence.
- Manage a caseload, prioritising assessments, reviews and discharges effectively.
- Maintain accurate records and support performance and quality reporting.
- Identify and escalate safeguarding concerns per policy.
- Contribute to multidisciplinary meetings, supervision, training, audits and service development.
- Use resources effectively to support service flow and timely discharge.
- Demonstrate NCA values of Care, Appreciate and Inspire.
