Job summary
As a Health Care Assistant you will support the 5-19 service to carry out the National Child Measurement Programme and deliver health promotion sessions alongside this.
You will record patient information in both electronic patient records and on the NCMP platform to ensure clear and accurate recording of measurements.
You will provide excellent customer service to all internal and external customers.
Main duties of the job
- Undertake the delivery of the national child measurement programme (NCMP) in named schools under the direction of the public health Nurse.
- Establish and maintain communication with patients, relatives and carers and the multidisciplinary team to be able to provide holistic patient care.
- Develop own skills and knowledge and provide information to others to help their development.
- Ensure own actions help to maintain quality.
- Assist in planning, delivering, and evaluating programmes of health promotion in schools and at community events/coffee mornings
- Assist in the use of technology for measurement, monitoring, and treatment.
- Ability to work flexibly and unsociable hours to meet the demands of the service.
- The post holder will be part of the Integrated Children's & Family Service and will work with all age groups 0-19 yrs. /25 (SEND) children to provide health promotion interventions as delegated by the public health Nurses within the Universal and or Targeted Teams. These interventions will be delivered primarily in Community locations, Nursery or Schools but may also be delivered in the Family Hub.
- Lead on group work with school aged children and support the delivery of health promotion in schools and other community locations, delivering appropriate Personal, Social, Health and Economic Education (PSHE).
