Job summary
The post holder will be a registered Nursing Associate and will practise in accordance with the NMC Code, Nursing Associate Standards of Proficiency, Trust policies and professional accountability requirements. They will recognise and work within the limits of their competence to provide high-quality, efficient and effective care to patients, supporting the Trust's commitment to improving the lives of people on the Isle of Wight and delivering care closer to patients' homes.
Working as part of Mental Health Services' frontline response, the post holder will support people referred for assessment, support and/or treatment. They will undertake risk assessments, complete appropriate referrals and inter-referrals, and plan and deliver intensive in-reach support where required, working collaboratively with patients, carers, colleagues and partner agencies to ensure safe and effective care.

Main duties of the job
To ensure and take responsibility for the provision of comprehensive assessments of needs and risks of people referred to the service.
To take responsibility for planning and the delivery of care to people requiring an acute and intensive response from this service.
To plan and take responsibility in collaborating with service users and their carers together with other services/agencies on how people's needs are going to be met in short-term and long-term situations.
To be active in the development of the service and continued development of professional practice and role.
To ensure that relationships and own behaviour are managed in line with the Trust's Equalities Framework competencies.
To ensure all communications are undertaken in a sensitive, confidential and professional manner.
Interdisciplinary working and liaison with internal and external agencies to promote holistic and person-centred care and inclusion.
To gather and process complex information from a variety of disciplines and agencies regarding initial referrals to the secondary mental health and social care services efficiently.
To ensure that rights, consent issues and capacity issues are clearly defined and shared.
To ensure that all legal frameworks pertaining to decisions made on behalf of another is carried out in a legally robust manner.
