This is a varied and rewarding role that requires a flexible approach to working. The successful candidate will support patients through a combination of clinic-based appointments, home visits and community-based activities across North West Wiltshire, helping patients to develop the skills and confidence needed to manage their diabetes effectively.

- Support the delivery of specialist diabetes services across community, domiciliary and hospital settings.
- Educate and train patients in the use of glucose monitoring systems, mobile applications and digital healthcare technology.
- Manage a delegated caseload, prioritising referrals for diabetes education and technical support.
- Deliver accredited Xpert structured education programmes for people living with Type 2 diabetes.
- Undertake delegated clinical duties under the supervision of Diabetes Specialist Nurses.
- Provide technical support for glucose monitoring devices, sensor insertion and data downloading.
- Maintain accurate clinical documentation and patient records in accordance with organisational policies.
- Participate in MDT working and contribute to high-quality, patient-centred diabetes care.
This role requires a motivated individual who can combine excellent patient care with strong digital and communication skills. You will be comfortable supporting patients to use healthcare technology while working closely with specialist clinicians to achieve positive outcomes.
- NVQ Level 3 or equivalent qualification.
- GCSE English Grade A-C (or equivalent).
- Ability and willingness to undertake additional training relevant to the role.
- Previous Healthcare Assistant experience.
- Experience working within a nursing or healthcare environment.
- Experience using computers, mobile phones, apps and email confidently.
- Strong understanding of IT systems and the ability to learn new technologies quickly.
- Experience educating, coaching or teaching patients.
- Experience working with individuals across different age groups.
- Excellent verbal and written communication skills.
- Ability to work collaboratively within a multidisciplinary team.
- Professional, patient-centred and empathetic approach.
- Experience supporting diabetes services or long-term condition management.
- Experience delivering presentations or education to groups.
- Knowledge of diabetes technologies such as glucose monitoring systems.
- Experience within community healthcare settings.
