Job summary

We have an exciting opportunity for an experienced Advanced Clinical Practitioner (ACP) to join the team at Newquay Community Hospital, delivering high-quality, patient-centred care within a community hospital setting.

This autonomous role involves managing patients with acute and chronic conditions, supporting admission avoidance, rehabilitation, patient flow and safe discharge planning. You will provide expert assessment, diagnosis, treatment and prescribing, ensuring patients receive the right care, in the right place, at the right time.

Working within a multidisciplinary team, you will collaborate with medical colleagues, nurses, therapists, primary care, social care and community services to deliver seamless care, particularly for older people with frailty, multiple long-term conditions and complex health needs.

The successful candidate will be an experienced ACP with independent prescribing qualifications, excellent clinical assessment skills and a commitment to service improvement. You will contribute to service development, lead quality improvement initiatives and support the education and development of colleagues.

Primarily based at Newquay Community Hospital, there may be occasional opportunities to support neighbouring community services in response to operational need.

Main duties of the job

As an Advanced Clinical Practitioner (ACP), you will provide expert autonomous clinical care within Newquay Community Hospital, working as part of a multidisciplinary team to deliver high-quality, person-centred care and support effective patient flow.

Key responsibilities include advanced assessment, diagnosis, treatment and management of patients with acute, chronic and complex conditions; managing an autonomous caseload, including admission assessments, patient reviews, diagnostic interpretation and independent prescribing within scope of practice.

You will work collaboratively with medical, nursing, therapy, primary care, social care and community teams to coordinate care and support safe discharge planning. The role includes developing care pathways for older people and those with multiple long-term conditions, reducing avoidable admissions and readmissions, and providing clinical leadership through specialist advice, supervision, support and role-modelling best practice.

The ACP will undertake advanced clinical procedures, monitor patient progress, identify deterioration and initiate appropriate investigations, interventions and referrals. You will contribute to service improvement, audit, research, governance and evidence-based practice, while supporting the education, mentorship and development of colleagues.
