Job summary
You will work within the Urgent Community Response team, working closely with other team members across the Intermediate Tier, supporting the service model for people requiring a timely response when in crisis relating to their health or social care across Bury.
You will provide clerical support to the team where appropriate and may be asked to work across the Tier at other venues within Bury.
You will have direct telephone contact with patients and referrers on a daily basis.
Main duties of the job
o Support the Urgent Community Response team in ensuring home visits are arranged in a timely and appropriate manner in accordance with the service criteria and targets.
o Liaise with other community service providers to collate relevant patient information adhering to information governance requirements. Collate all appropriate documentation to support the home visits booked with the clinical team.
o Accurately complete and action documentation as requested by the clinical team members.
o To create and modify documents using advanced Microsoft Office packages
o Organise the administration aspects of coordinating meetings and events.
o To attend meetings and take minutes where necessary.
o To provide word processing support as required, including letters, reports and e-mails
o To maintain confidentiality and monitor computerised and manual records
o To collate and provide statistical information/reports as required e.g. performance management, FOI requests etc.
o Maintain a staff/team record to support the maintenance of an up to date duty rota and other service cover as necessary.
