Job summary

This is a blended role, both elements of social prescribing and health & wellbeing coach are required.

The post is within Preston North & East PCN and hosted by Issa Medical. You will be required to work across more than one practice as well as some outreach working within the community.

The PCN covers a large and diverse population, both culturally and economically. Preston has areas of high deprivation and as a PCN we have a Targeted Demand Management approach within our health inequalities agenda.

as well as 1:1 targeted and generic referrals from practices, we work closely with colleagues across both the voluntary and public sector on joint initiatives.

General Practice is moving towards a neighbourhood model of working and the work carried out within our health inequalities teams plays an important aspect as we work towards this.

This is an exciting opportunity to be part of a wider team across both Preston PCNs covering the Heart of Lancashire GP Federation footprint.

Main duties of the job

You will be part of our health inequalities team, the role will be a mix of social prescribing and health and wellbeing coach.

Our approach to social prescribing is to support patients referred via a clinician with access and signposting to empower them to make better choices and enable them to access services who can support addressing their health and social needs.

As part of our Targeted Demand Management strategy, this will require more intense support of particular cohorts of patients, offering them more 1:1 advice, guidance and assistance to achieve an improvement in their health.

There will also be a requirement to liaise with community groups, attend events and offer general health advice and information. This will require simply tests such as blood pressure, height & weight etc. All equipment and training will be provided.

About us

Preston North & East is a large Primary Care Network (PCN) made up of 8 practices and covers a registered population of approximately 108,000 patients.

We have recently become part of the newly formed Heart of Lancashire GP Federation and work closely with our neighbouring PCNs, both in Preston and across Central Lancashire.

Issa Medical is the lead practice for the PCN and will host this role. We have 2 practices, 1 in Deepdale and1 in Fulwood. the remaining PCN practices are in the Ribbleton, Avenham, Longridge and Fulwood areas.

You will be required to work across more than one practice and attend events across Preston.

Job description

Job responsibilities

The post holder will play a key role in supporting patients to improve their health, wellbeing and independence through a personalised, person-centred approach.

Working closely with GP practices, community services, voluntary organisations and the wider integrated Neighbourhood Team. The role will support patients to identify goals, develop self-management skills access community resources and address the wider determinants of health.

The role will focus on supporting frail, vulnerable and at risk patients through both proactive and reactive interventions, helping to reduce health inequalities, prevent crisis situations and avoid unnecessary hospital admissions.

The post holder will work within agreed protocols and their scope of practice, escalating concerns to registered clinicians where appropriate.

This is a non-clinical role that acts as a bridge between primary care services and the realities patients face in their daily lives, ensuring support is based on what matters most in individuals.

Person Specification

Qualifications

Essential

  • GCSE grade 4/C or above (or equivalent) in English and Mathematics
  • NVQ Level 3 qualification or equivalent experience
  • Evidence of continuous professional development

Desirable

  • Level 2 or Level 3 Health Coaching qualification
  • Social Prescribing, Personalised Care or Coaching qualification
  • Willingness to undertake further training relevant to the role
