Job Introduction We have an opportunity for an Advanced Recovery Practitioner role to support the RECONNECT team based at County Hall. The RECONNECT service aims to improve continuity of care to vulnerable people leaving prison by supporting them prior to release and their subsequent transition to community-based services.
