Job Purpose

To provide a high-quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education.

Accountable for delivering evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients.

Main Duties & Responsibilities

Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being including being able to prescribe

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols

To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems

Take responsibility for national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule

Provide travel health advice and administer appropriate vaccinations

Treat patients with minor injuries, assess and create treatment plans for patients with minor injuries alongside ANP

Provide directly and assist GPs with sexual/womens health

Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs

Lead and support chaperones and ensure recognised training has taken place.

Lead and take responsibility for health promotion to help educate patients.

Select, carry out and interpret results of specific tests.

Administration/supply of medicines on an individual basis through PGD.

To act as contact for third sector agencies and other organisations to assist holistic patient care

Responsible for infection control policies, ensuring compliance with current guidelines. Must be willing to undertake lead site-based IPC roles if required and support the Nursing team, ensuring IPC audits are completed by themselves or their deputy as appropriate.

Demonstrates awareness of safeguarding policies and procedures, including the ability to identify safeguarding concerns and promptly escalate them in line with organisational protocols. Maintains up-to-date knowledge of safeguarding guidance and is able to identify and refer appropriately to the designated safeguarding lead(s) within the practice

To maintain accurate patient records and enter onto the computer using agreed Read Codes

Ensure accurate completion of all necessary documentation associated with patient health care

Take responsibility for Qof and clinical areas of LES/DES, liaising with staff to ensure the practice achieves targets

To support clinical effectiveness by co-ordinating and leading the practice clinical audits and research project.

To contribute to the preparation of any Practice development plans.

To maintain own annual registration with the NMC and adhere to Code of Professional Conduct, including personal responsibility for revalidation.

To maintain a safe and therapeutic environment for all PHC staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to.

To always promote a professional approach by attire and attitude to patients and staff both in and out of the Practice.

To maintain own continuous professional development and ensure PDP kept up to date in line with the requirements of the Practice.

Attend and participate in staff and Practice meetings and in-house clinical training sessions.

To be flexible to work across multi sites.

Person Specification

Experience

Essential

• Proven experience in delivering nursing care within a primary care or community setting.
• Experience in managing chronic disease clinics and delivering national screening programmes (e.g., cervical cytology).
• Experience in immunisation and vaccination delivery, including travel health advice.
• Experience in minor injury assessment and treatment or willingness to develop this skill.
• Experience of using Patient Group Directives (PGDs) for medication administration.
• Experience in clinical audit and quality improvement initiatives
• Experience supporting and mentoring junior staff such as APs and HCAs is desirable.

Qualifications

Essential

• Registered Nurse with current Nursing and Midwifery Council (NMC) registration
• Qualification and/or experience in general practice nursing or community nursing
• Current immunisation certification or willingness to complete
• Evidence of ongoing professional development and commitment to revalidation

Knowledge & Skills

Essential

• Strong clinical assessment, diagnostic, and treatment planning skills.
• Ability to manage and deliver chronic disease management in line with evidence-based practice and QOF requirements.
• Understanding of safeguarding adults and children, and ability to apply this in practice.
• Excellent record-keeping skills with ability to maintain accurate and confidential patient data using clinical IT systems and Read Codes.
• Knowledge of infection control procedures and health and safety regulations.
• Strong communication and interpersonal skills to engage effectively with patients, carers, and multi-disciplinary teams.
• Ability to provide sensitive information and support to patients and their families (e.g., breaking bad news).
• Organisational skills with the ability to manage multiple priorities and maintain robust call and recall systems.
• Proficiency in delivering health promotion and patient education.
