Job summary
We are seeking an experienced and motivated Practice Nurse with either a level 6 diploma or have a post grad certificate in a long term condition to join our clinical team at Broomfield Park Medical Centre. This role offers an opportunity for a skilled nurse to work within a supportive general practice environment, delivering a broad range of nursing services to our registered patient population in Spon End area of Coventry.
The successful candidate will work alongside GPs and the wider practice team to provide planned and preventative care, support patients with long-term conditions and contribute to improving health outcomes within our community.
This is a varied role requiring clinical expertise, good judgement and the ability to work independently while maintaining effective communication with colleagues and patients. The ideal candidate will be committed to delivering excellent standards of nursing care and supporting the continued development of primary care services.

Main duties of the job
The Practice Nurse will provide a range of clinical services to patients registered with Broomfield Park Medical Centre, working as part of the wider healthcare team to deliver safe and effective care.
The post holder will undertake nursing assessments, develop care plans and support the management of patients with long-term conditions, including diabetes, respiratory disease, cardiovascular disease and hypertension. They will deliver preventative healthcare services including immunisations, cervical screening, NHS health checks and health promotion advice.
