Job summary

Are you a compassionate and motivated Registered Nurse looking for a rewarding role where you can make a real difference to patients in their own homes? We are seeking enthusiastic Band 5 Community Staff Nurses to join our well-established Out of Hours Community Nursing Service, providing high-quality care across East Cheshire.

As part of our supportive and experienced team, you will deliver a wide range of nursing interventions, helping patients remain safely at home and avoiding unnecessary hospital admissions. No two shifts are the same, offering opportunities to develop your clinical skills, autonomy, and decision-making in a diverse community setting.

We understand the importance of work-life balance and can offer a flexible approach to working patterns. Whether you are looking for just one shift per week or several shifts, we would be delighted to hear from you. We currently have a range of evening shifts available throughout the week, including 18:00 to 22:00 and 17:00 to 22:00 shifts, enabling you to fit community nursing around your existing commitments.

Main duties of the job

As a Band 5 Community Staff Nurse within the Out of Hours Community Nursing Service, you will provide high-quality, patient-centred nursing care to individuals in their own homes and residential care settings across East Cheshire.Working as part of a supportive multidisciplinary team, you will be responsible for assessing, planning, implementing and evaluating care, ensuring patients receive timely and effective nursing interventions during the out of hours period.

Key responsibilities include:

Delivering a wide range of community nursing interventions, including wound care, palliative and end of life care, catheter care and medication administration.
Undertaking holistic patient assessments and using clinical judgement to identify changing healthcare needs.
Supporting patients to remain safely at home, preventing unnecessary hospital admissions and facilitating early discharge where appropriate.
Responding to urgent referrals and prioritising workload effectively to ensure safe and timely care delivery.
Working collaboratively with patients, families, carers, GP services, acute hospitals, care homes and wider multidisciplinary teams.
Maintaining accurate and contemporaneous clinical records in line with Trust policies and professional standards.
Promoting health, wellbeing and patient independence through education and support.
Participating in service development, clinical supervision, mandatory training and continuous professional development activities

Registered Nurse to degree/diploma level
Current registration with NMC
Evidence of recent professional development in an up-to-date portfolio

Non- Medical Prescriber Teaching/Assessing qualification

Experience

Recent previous experience within a comparable role/transferable skills
Demonstrable knowledge of assessment and therapeutic interventions in area of practice
Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
Ability to prioritise and organise workload effectively
Experience of using electronic patient/service user record systems
Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail or written communication
