Job summary
We are looking for a dynamic, compassionate and motivated Band 6 Therapist to join our Integrated Front Door Team, an innovative service at the forefront of improving urgent care pathways, supporting safe patient transitions and reducing system pressures.
This role provides an exciting opportunity to support safe, timely and effective patient care at the front door, with a focus on preventing unnecessary hospital admissions, reducing length of stay and promoting proactive, person-centred care through early therapy assessment and intervention. Working as part of a fully integrated multidisciplinary team including consultants, GPs, pharmacists, nurses, therapists and healthcare support workers, you will undertake holistic therapy assessments, support clinical decision-making, contribute to admission avoidance and facilitate timely discharge planning across acute, community and partner services.
We are seeking an experienced therapist who is confident working within a fast-paced environment and shares our commitment to delivering high-quality, person-centred care. You will play an important role in supporting patient flow, promoting independence, identifying appropriate onward pathways and contributing to a responsive and resilient Integrated Front Door service.
Main duties of the job
The post holder will undertake holistic therapy assessments for patients presenting at the Integrated Front Door, identifying functional ability, rehabilitation potential, risks and immediate support needs. They will provide timely, evidence-based therapy intervention to support admission avoidance, early discharge planning and safe transfer onto appropriate community pathways, working collaboratively with the multidisciplinary team to support clinical decision-making, patient flow and safe, person-centred care. The role will include identifying and facilitating referrals to appropriate onward services, including Urgent Community Response, Virtual Ward, reablement, rehabilitation pathways, community therapy services, social care and voluntary sector support where appropriate. The post holder will promote independence, functional recovery and prevention-focused care, manage a delegated caseload in a fast-paced front door environment, contribute to board rounds, MDT discussions and discharge planning, maintain accurate and timely clinical records, work within professional scope of practice, and support service improvement and pathway development across acute, community, local authority and partner services.
