Role description

Job summary Are you passionate about patient care and helping patients to remain in their own homes and prevent hospital admissions? Our service currently operates 08:00 to 19.00 Monday - Friday and 08:00- 16:00 at weekends and bank Holidays. There is an aspiration that the hub operating hours will extend to working 08:00-20:00 at some point in the future.
