Role description
Job summary
To act as the specialist lead for housing within the service, providing expert oversight, co-ordination and decision-making in relation to housing pathways, complex cases and discharge planning.
The postholder will be responsible for ensuring that housing is embedded into clinical pathways from admission through to discharge, enabling timely and safe transitions of care. The role will involve working autonomously to manage complex and high-risk housing situations, providing specialist advice to multidisciplinary teams and partner agencies, and leading the development and implementation of housing-related processes and improvements.The postholder will operate with a high degree of professional autonomy, interpreting legislation, policy and guidance to make decisions in complex, sensitive and often ambiguous situations.
There is a requirement to work flexibly across locations, Monday to Friday, 9am to 5pm.
Main duties of the job
The Housing Support Lead will provide specialist leadership for housing across inpatient and community pathways, ensuring housing needs are identified and addressed from the point of admission to support safe and timely discharge. The postholder will manage a small caseload of the Trust's most complex housing-related discharge cases, providing expert advice to multidisciplinary teams, coordinating with Local Authorities, housing providers and partner agencies, and leading the resolution of barriers such as homelessness, tenancy issues and access to suitable accommodation. The role will also drive service improvement by embedding housing into discharge pathways, monitoring housing-related delays, developing operational processes, and providing strategic oversight to reduce unnecessary length of stay, improve patient flow and support the Trust's discharge performance.
