Role description

Job summary

An exciting opportunity has arisen for an experienced HCSW to join the home therapies peritoneal dialysis team. This progressive, passionate team strive to support patients to self manage their peritoneal dialysis treatment at home. The role will involve management of patients attending clinics in settings, as well as home visits across Dorset and Somerset. This role would suit someone who is dedicated to providing truly holistic care to their patients.

Main duties of the job

Teaching patients to independently manage their peritoneal dialysis across the geographical area of Dorset and Somerset.

Carry out assessments of patients and routine tests required for management of peritoneal dialysis; and escalate concerns accordingly.

Be comfortable working as a team, but also be able to manage time effectively when working independently.

Driving licence and confidence in driving across the geographical caseload patch is essential, as involves home visits.

Maintain stock levels.

Job responsibilities

The role will involve being confident to communicate with patients, relatives, MDT and primary care regarding patients care; have excellent documentation skills and attention to detail when recording in patients notes and written communication. Knowledge of the care of renal patients would be an advantage, but not essential as full competency based training will be given, in a supportive, dynamic team environment.
