We are seeking to recruit motivated practitioners with initiative and drive who are keen to contribute to the further development of our integrated children's health service. The Children's Health Team work within Lincolnshire County Council very closely with Children's Services (Children's Centres, Early Help, Social Care and Family Hubs) and other agencies. Our aim is to improve outcomes and reduce health inequalities for children, young people and their families in our care. Due to our integrated ways of working, we are able to provide a comprehensive package of care to ensure that our children are able to achieve their full potential. We deliver our service in different environments such as within the home, childrens centres and schools.
Full time posts in East Lindsey North and South, Gainsborough, Lincoln South, Spalding Rural and Boston.
22.5 hour posts in Sleaford and Boston.

It is essential that you are a keen public health advocate who can demonstrate delivery of the healthy child programme. You will need to have the ability to work autonomously and as part of an integrated skill mix team.You need to have excellent communication skills and experience of working in partnership across organisational boundaries.You will need experience and knowledge of safeguarding children and families.You will need to be able to drive.
