Job responsibilities

1. Clinical Responsibilities

In accordance with the agreed practice timetable, the post-holder will:

• Undertake face-to-face, telephone, video and electronic consultations, as appropriate.
• Conduct home visits where clinically required and agreed within the practice arrangements.
• Assess, diagnose and manage patients presenting with undifferentiated, undiagnosed and established medical conditions.
• Make professional and autonomous clinical decisions in relation to patient presentations, including those referred by other members of the healthcare team.
• Provide appropriate clinical assessment, treatment, follow-up and referral.
• Develop and review individualised care plans in partnership with patients and in accordance with current evidence-based clinical guidance and practice protocols.
• Undertake appropriate screening, health promotion and identification of disease risk factors and early signs of illness.
• Provide appropriate counselling, advice and health education to patients and carers.
• Prescribe safely and appropriately in accordance with current legislation, national guidance and the practice prescribing policy/formulary, using generic prescribing where clinically appropriate.
• Review and authorise repeat prescriptions and deal with medication-related queries within an appropriate timescale.
• Complete clinical correspondence, referrals, reports, medical forms and other administrative work arising from consultations in a timely manner.
• Maintain accurate, clear and contemporaneous clinical records in accordance with professional and practice standards.
• Ensure appropriate coding and recording of clinical information.
• Participate in clinical audits and other activities designed to monitor and improve the quality of care.
• Work collaboratively with GPs, nurses, pharmacists, healthcare assistants, administrative staff and other members of the multidisciplinary team.
• Recognise and act appropriately on safeguarding concerns involving children and adults at risk.
• Escalate urgent or clinically significant concerns appropriately.
• Undertake any other duties reasonably expected of a GP working within primary care.

Experience

Essential

• *Experience of working in primary care, with broad clinical knowledge and the ability to manage a wide range of acute and chronic conditions.
• *Commitment to safeguarding, clinical governance, patient safety and high-quality, patient-centred care.
• Excellent communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team.

Qualifications

Essential

• * GMC registration with a licence to practise.
• * Inclusion on the NHS England Medical Performers List.
• * Completion of GP Specialty Training (MRCGP or equivalent).
• * Evidence of appropriate postgraduate training and continuing professional development.
