Role description
Job summary
We are looking for an organised, detail-oriented and proactive individual to join our busy practice team in a dual role encompassing both Office Administration and Clinical Coding responsibilities.
This varied position plays a key role in ensuring the accuracy of patient records, supporting clinical and administrative teams, and contributing to an excellent patient experience. Responsibilities include processing and coding patient correspondence, maintaining electronic patient records, supporting patient registration processes, handling administrative and secretarial tasks, and providing telephone support to patients on a rota basis.
You will:
- Have experience working in a busy administrative environment.
- Possess excellent organisation, communication and time management skills.
- Be confident using Microsoft Office applications.
- Have strong attention to detail and be comfortable handling confidential information.
- Be able to prioritise workloads and work effectively both independently and as part of a team.
- Ideally have experience working within the NHS or Primary Care setting and knowledge of clinical coding systems.

Main duties of the job
Duties and Responsibilities:
Scan patient related documents onto their medical record.
Ensure incoming letters are scanned onto the patient record
Identify and appropriately action relevant clinical information contained in patient correspondence.
Code or free text the clinical information on to the patient record.
Summarisation of patients notes when requested.
Ensure security of data at all times.
Process patient records for Medical Reports etc
Dealing with patients for access to medical records.
Create patient online registrations, generating access levels and dealing with day-to-day enquiries.
To be responsible for patient records coming in and out of the practice, in accordance with practice procedures.
Processing and removing deceased patients and out-of-area patients.
Processing new patient registrations.
Reviewing medical records and producing an accurate summary of the patients medical history, read coding information in line with procedures.
Sort all incoming patient related mail.
Scan patient related documents onto their medical record using read codes as agreed by the clinical team.
Code the clinical information on to the patient record.
To provide an efficient copy/audio typing and word processing service for all Clinicians as required.
To make appointments and bookings for patients as required.
Provide secretarial support to Palliative Care and Operations meetings.
Support the Care Navigation Team a rota basis.
