Job summary We are offering an exciting opportunity to join Discharge to Access (D2A) Complex Assessment team as a Discharge Facilitator based in Stoke on Trent You will have responsibility for assessing and tracking patients requiring a discharge to assess (D2A) bed-based service within a timely manner in a non-acute setting and closely work with the multi-disciplinary team and external agencies to ensure a robust assessment and a safe discharge.
