Job summary

Are you an experienced and motivated nurse passionate about delivering outstanding community-based care?

An exciting opportunity has arisen to join the Frailty Virtual Ward, an innovative partnership between Leicestershire Partnership NHS Trust, University Hospitals of Leicester and Adult Social Care. The service provides a safe alternative to hospital admission for people with moderate to severe frailty, delivering high-quality care in patients' own homes through a combination of clinical assessment, face-to-face interventions and remote monitoring.

As a Band 6 Frailty Community Nurse, you will assess, treat and manage patients with complex acute needs, supporting recovery, independence and timely care. You will use clinical judgement to monitor patients, coordinate care, respond to changes in condition and escalate concerns appropriately.

Working closely with Advanced Clinical Practitioners, Geriatricians, Allied Health Professionals, Social Care colleagues and the wider multidisciplinary team, you will help deliver responsive, patient-centred care while reducing avoidable hospital admissions. You will also contribute to service development and quality improvement within this evolving model of care.

The service operates 08:00-18:00, including weekends and bank holidays on a rota basis.

You will prioritise workload, coordinate care, respond to urgent or deteriorating situations, and maintain accurate clinical records. Working closely with patients, carers, families and partner organisations, you will promote integrated, holistic care and positive patient outcomes.

The role includes contributing to service development, quality improvement and the implementation of evidence-based practice, policies and clinical guidelines to enhance care delivery.

You will act as a professional role model, supporting the learning and development of students and colleagues through supervision and education. Maintaining professional standards, completing mandatory training, participating in clinical supervision and CPD, and ensuring compliance with NMC, Trust and regulatory requirements are essential aspects of the role.
