Role description
Job summary
<p><strong>Make a meaningful difference every day by delivering expert care that enables patients to remain safe, <span class="single_desc_int_gb_word">independent, and <span class="single_desc_int_gb_word">supported</span> within their own homes and communities</span>.</strong></p><p>As a Band 6 Community Nurse / District Nurse, you will play a key role in delivering high-quality, person-centred care within the community setting. Working as part of a multi-professional team, you will be responsible for the assessment, planning, coordination, and delivery of clinical care that meets the needs of patients and the service.</p><p>You will work autonomously within your scope of practice, using your clinical expertise, professional judgement, and evidence-based knowledge to make informed decisions and deliver safe, effective care. As a registered practitioner, you will be accountable for your actions, recognising the limits of your <span class="single_desc_int_gb_word">competence and seeking <span class="single_desc_int_gb_word">support</span> where appropriate</span>.</p><p>The role also includes supervising and <span class="single_desc_int_gb_word">supporting junior staff, <span class="single_desc_int_gb_word">support</span> workers, and students, helping to develop future healthcare professionals</
