Role description
Job summary
Due to investment and development in our new Community Response Hubs we are looking to recruit a Part time Community Nurse Band 6, Clinical Lead working 28.5 hours per week over three days.
Based at Tarporley Hospital visiting the patients of the Rural Response Hub area. Travel is an important part of this role. Access to transport for work purposes is essential.
Main duties of the job
The post holder will be a clinical resource, providing a visible clinical profile, having advisory, leadership and clinical oversight of the caseload.
The team focus is on prevention of unnecessary acute hospital admission, End of Life Care and Facilitating discharge from hospital.
By working collaboratively with the wider healthcare team we ensure person centred care for those patients whose preferred place of care is at home.
A high level of communication is essential for the role, with Team work at the core of our practice.
