Wootton Medical Centre is excited to offer an opportunity for an experienced and motivated Advanced Nurse Practitioner to join our friendly and supportive team, up to 20 hours per week. We are looking for a passionate and patient-focused clinician who enjoys working as part of a team while also being confident in autonomous practice. You will be committed to delivering excellent care, maintaining high clinical standards, and helping us continue to improve the services we provide to our local community.
The ideal candidate will have experience in compression bandaging, cervical screening, immunisations and injections, phlebotomy, and wound care, although training and development opportunities may be available for the right candidate.

We are seeking an experienced and enthusiastic Advanced Nurse Practitioner to join our multidisciplinary clinical team. This is an exciting opportunity for a motivated clinician with expertise in respiratory disease management and diabetes care to play a key role in shaping and delivering services that make a genuine difference to the health and wellbeing of our patients.
Main duties of the job
Provide autonomous, high-quality clinical care to patients of all ages within a busy GP practice
Assess, diagnose, treat and manage patients presenting with acute, chronic and complex health conditions
Prescribe and review medications in line with national guidance, clinical protocols and scope of practice
Deliver evidence-based care using advanced clinical assessment and diagnostic skills
Lead and develop specialist respiratory and diabetes services
Manage patients presenting with urgent and emergency healthcare needs
Promote health improvement, disease prevention and self-management to support positive patient outcomes
Undertake minor illness management, wound care, immunisations and other nursing duties appropriate to competence and training
Work collaboratively within a multidisciplinary team to ensure safe, effective and patient-centred care
Contribute to clinical governance, quality improvement, audits and service development initiatives
Support the education, supervision and development of colleagues and junior staff
Maintain accurate clinical records and ensure compliance with professional, legal and organisational standards
Act as a clinical leader, championing best practice and continuous improvement across the practice

Our multidisciplinary workforce includes GPs, Practice Nurses, Healthcare Assistants, Clinical Pharmacists, Physiotherapists, Social Prescribers, Paramedics, and Reception and Administration Teams.
