Job description

Job responsibilities

Provide individual support to service users, through face-to-face and telephone contact, to explore ways of improving their mental health and wellbeing.

Support service users to identify and make practical changes to their environment, routines and lifestyle to improve their health and wellbeing, including making informed choices around physical activity, healthy eating and mental wellbeing, and accessing relevant community, voluntary sector and social activities and services.

Support access to wider community activities and support as part of an individuals plan.

Work collaboratively and maintain effective communication with colleagues across Sheffield Mind, the PCS clinical team and other health, social care, voluntary and community services.

Safely and effectively manage own caseload under supervision the wider PCS ADHD Clinical and operational staff.

To work within competencies, autonomy and capacity and seek guidance and support appropriately.

Accurately record contacts, interventions, outcomes and other relevant information in accordance with organisational requirements, ensuring performance information is recorded in a timely manner.

Contribute to the development and delivery of effective working practices within the service, supporting the achievement of agreed outcomes and service objectives.

Work within the competencies, autonomy and capacity of the role, seeking appropriate guidance and support from the wider PCS ADHD clinical and operational team when required.

You will follow PCS policies and relevant professional standards, including those relating to information governance, safeguarding, confidentiality, health and safety and EDI.

Person Specification

Qualifications

Essential

• NVQ Level 3 in Health and Social Care or equivalent

Experience

Essential

• Significant experience of working in a support worker or social prescriber role
• Understanding of the impact that ADHD symptoms can have on mental health, wellbeing and everyday life
• Demonstrable experience of providing 1-1 support to individuals to improve their health and wellbeing
• Demonstrable experience of planning and delivering structured, clinic-based interventions such as goal setting, action planning and behaviour/lifestyle change

Desirable

• An understanding of the barriers faced by people experiencing health inequalities
• Basic understanding of the Mental Health Act and its use in practice or equivalent health and social care legislation

Skills and Competencies

Essential

• Excellent communication and interpersonal skills, with the ability to build effective relationships with service users
• Ability to recognise when someone requires additional support and appropriately escalate or refer
• Able to work independently and manage and prioritise a caseload effectively
